Hair Colour Questionnaire

A short questionnaire that helps a colourist design a bespoke formula ahead of the appointment. It asks about the current shade, any earlier bleaching, hair length and whether the client prefers a particular stylist. Attached photos show both the starting point and the look the client has in mind.
